Application Effect of"Internet+"Out-of-Hospital Intervention Strategy Based on Risk Stratification in Patients with Hypertension

Abstract
Objective To explore the application effect of"internet+"out-of-hospital intervention strategy based on risk stratification in patients with hypertension.Methods A total of 120 patients with hypertension who were treated in the Department of General Practice,Inner Mongolia People's Hospital from September 2024 to February 2025 were selected as the research objects.They were divided into the control group and the observation group by random number table method,with 60 cases in each group.Patients in the control group received routine post-discharge intervention,and patients in the observation group received"internet+"out-of-hospital intervention strategy based on risk stratification.Both groups were continuously intervened for 6 months.Blood pressure control,self-management ability[assessed by Hypertension Patients Self-Management Behavior Rating Scale(HPSMBRS)],treatment compliance[assessed by Therapeutic Adherence Scale for Hypertensive Patients(TASHP)]and quality of life[assessed by Quality of Life Instruments for Chronic Diseases-Hypertension(QLICD-HY)]were compared between the two groups before intervention and at 6 months of intervention.Results At 6 months of intervention,the systolic blood pressure and diastolic blood pressure in the observation group were lower than those before intervention,respectively,and those in the observation group were lower than those in the control group(P<0.05).At 6 months of intervention,the blood pressure control rate of the observation group was higher than that of the control group(P<0.05).At 6 months of intervention,the scores of HPSMBRS,TASHP and QLICD-HY in the observation group were higher than those before intervention,respectively,and those in the observation group were higher than those in the control group(P<0.05).Conclusion The"internet+"out-of-hospital intervention strategy based on risk stratification can effectively reduce blood pressure of patients with hypertension,and improve the patients'blood pressure control compliance rate,self-management ability,treatment compliance and quality of life.关键词
