Clinical application outcomes of frozen-thawed testicular aspirate tissue containing only immotile sperm observed under microscopy

Abstract
Objective:To evaluate the clinical application value of conventional cryopr-eservation for testicular samples with only immotile sperm by analyzing embryonic outcomes,pregnancy outcomes and neonatal outcomes.Methods:A retrospective study was conducted to collect clinical data of 54 patients diagnosed with suspected obstructive azoospermia(OA)who underwent diagnostic Testicular Sperm Aspiration(TESA)at the Reproductive Genetics Cen-ter,The First Affiliated Hospital of University of Science and Technology of China,from August 2022 to October 2024.All patients underwent cryopreservation of testicular sperm.Among them,17 cases had only immotile sperm observed under the microscope(the immotile sperm group),and 37 cases had motile sperm(the motile sperm group).After thawing,intracytoplas-mic sperm injection(ICSI)was performed.Indicators including fertilization rate,good-quality embryo rate,clinical pregnancy rate,and neonatal outcomes were observed.Results:After tha-wing of testicular sperm,all samples were treated with sperm agonist.Except for 1 case in the immotile sperm group where no motile sperm was found,the remaining 53 frozen-thawed sam-ples had sufficient motile sperm for ICSI.The fertilization rate(71.96%vs78.77%),blasto-cyst formation rate(52.94%vs 49.77%),clinical pregnancy rate(46.67%vs 42.31%),miscarriage rate(7.14%vs 13.64%),the first embryo transfer cycle live birth rate(43.75%vs 37.5%),and cumulative live birth rate(81.25%vs 56.25%)showed no statistically sig-nificant differences between the immotile sperm group and the motile sperm group(P>0.05).Additionally,there were no significant differences in neonatal outcomes such as neonatal length,weight,and Apgar score between the immotile sperm group and the motile sperm group(P>0.05).Conclusion:For OA patients undergoing diagnostic TESA,conventional cryopreservation of tissue samples containing only immotile sperm is one of the optional strategies for fertility preservation in OA patients.关键词
