体外膈肌起搏及其联合疗法治疗慢性阻塞性肺疾病的研究进展
Research progress of external diaphragm pacing and its combination therapy in the treatment of chronic obstructive pul-monary disease

Abstract
Chronic obstructive pulmonary disease(COPD)is a chronic respiratory disease characterized by persis-tent airflow limitation and progressive dyspnea.Its global incidence continues to rise and has become a serious public health problem.Existing treatments mainly rely on drug interventions,but they are limited in improving respiratory muscle function and improving quality of life.It is urgent to explore new non-drug treatment strategies.This article aims to systematically review the research progress of external diaphragmatic pacing(EDP)in the management of COPD,focusing on evaluating its clinical effects and potential mechanisms of independent application and combined intervention,and providing new treatment ideas and evidence-based basis for clinical practice.Relevant Chinese and English literature published in PubMed,Web of Science,and CNKI databases from 2019 to 2025 were systematical-ly searched,and keywords including"external diaphragmatic pacing","chronic obstructive pulmonary disease","combined therapy"were screened to summarize the application status and research results of EDP.EDP can en-hance diaphragmatic contraction function,improve lung function indicators(such as FEV1,FVC),regulate respira-tory drive,and optimize ventilation-perfusion ratio through transcutaneous electrical stimulation of the phrenic nerve,thereby improving exercise endurance and quality of life.Combined with pulmonary rehabili-tation,non-invasive ventilation,traditional Chinese medicine exercises,etc.,it has a synergistic effect,which helps to enhance respiratory muscle endurance,re-duce hospitalization rate,improve psychological state,etc.However,its clinical application still faces challenges such as individualized parameter setting,long-term efficacy e-valuation and cost-effectiveness analysis.EDP,as a promising non-drug intervention,shows good potential in the treatment of COPD.In the future,we should strengthen the basic mechanism research,optimize the combined inter-vention strategy,and carry out high-quality,multi-center randomized controlled trials to promote its standardized and individualized clinical application.关键词
