县域公立医院与养老机构合作模式的探索
Exploration of collaboration models between county-level public hospitals and elderly care institutions

Abstract
Collaboration between county-level public hospitals and elderly care institutions is a critical strategy for enhancing the capacity of integrated medical and elderly care services at the grassroots level.As a rep-resentative case,The First People's Hospital of Xiaoshan District in Hangzhou has actively partnered with local elderly care institutions through the establishment of a medical consortium,targeted resource support,implementa-tion of professional skills training,dissemination of health science knowledge,and organization of academic ex-changes.This initiative has preliminarily achieved the integration of medical and elderly care resources,the estab-lishment of a two-way referral mechanism,capacity building for healthcare personnel,the popularization of health knowledge,and the development of an academic framework.However,several challenges remain,including the absence of a collaborative governance mechanism,mismatches between service supply and demand,insufficient talent development,limited diversity in health education approaches,and a lack of research momentum.To ad-dress these issues,it is essential to deepen the concept of shared responsibility and governance,construct smart medical-elderly care integration solutions,establish a comprehensive talent cultivation system,build platforms for translating research into practice,and improve the quality supervision and feedback loop.These strategies aim to promote high-quality development of integrated medical and elderly care services.关键词
