Advantages and challenges of circulating tumor DNA testing after radical resection of colorectal cancer

Abstract
In recent years,significant progress has been made in using circulating tumor DNA(ctDNA)detection to monitor minimal residual lesions(MRL)after radical surgery for colorectal cancer(CRC),guiding treatment decisions and monitoring recurrence and metastasis.Currently,a large number of observational or prospective clinical studies support that ctDNA detection can be used to stratify the risk of clinical recurrence in CRC,guide individual postoperative auxiliary treatment decisions(upgrade or downgrade therapy)after radical resection of CRC,while enabling earlier prediction of recurrence risk through imaging.The feasibility of selecting treatment regimens based on postoperative ctDNA status is increasingly robust,simultaneously enhancing the precision of clinical risk stratification for CRC recurrence.Therefore,ctDNA detection is expected to change the existing treatment framework of CRC and achieve individualized stratified precision therapy,but it still needs to be further confirmed by follow-up high-quality prospective large-scale randomized controlled trials.This article will summarize the definition and clinical significance of MRD,the main indications and common techniques of ctDNA-MRD testing,and the relevant clinical studies,application prospects,and technical difficulties of ctDNA testing after radical CRC surgery.关键词
