Screening of differential intestinal bacterial genera and mediating effect analysis between quality of life and depression in colorectal cancer patients

Abstract
Objective To screen differential intestinal bacterial genera between depressed and non-depressed patients with colorectal cancer(CRC),and to explore the mediating role of bacterial abundance in the relationship between quality of life and depression severity.Methods A total of 98 CRC patients admitted to Chun'an First People's Hospital from August 2021 to August 2023 were enrolled.According to Self-Rating Depression Scale(SDS)scores with a cut-off of 53,patients were divided into a depression group(n=24)and a non-depression group(n=74).Quality of life was assessed using the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire-Core 30(EORTC QLQ-C30).Fresh stool samples were collected for 16S rRNA gene sequencing to compare gut microbiota profiles between groups.Pearson correlation analysis was used to examine relationships among quality of life,differential bacterial abundance,and depression scores.Mediation analysis was performed to evaluate the role of bacterial abundance in the association between quality of life and depression in CRC patients.Results The depression group showed significantly lower scores in all EORTC QLQ-C30 domains and total score(all P<0.05).Gut microbiota diversity indices(Shannon,Chao1,and ACE)were higher in the depression group(all P<0.05),while the relative abundances of Coprococcus,Dialister,Faecalibacterium,Prevotella,and Bacteroides were lower(all P<0.05).Pearson analysis revealed that the total score of EORTC QLQ-C30 were positively correlated with Coprococcus abundance(r=0.743,P<0.05)and negatively correlated with SDS scores(r=-0.517,P<0.05).Coprococcus abundance was also negatively correlated with SDS scores(r=-0.502,P<0.05).Mediation analysis indicated that Coprococcus abundance partially mediated the relationship between the total score of EORTC QLQ-C30 and SDS scores,with a mediating effect of-0.376,a total effect of-4.593,accounting for 8.19%of the total effect.Conclusion Quality of life is associated with depression severity in CRC patients,and Coprococcus abundance plays a partial mediating role in this association.关键词
