中西医治疗射血分数正常的心力衰竭研究进展
Research Progress on the Treatment of Heart Failure with Preserved Ejection Fraction Using Integrated Traditional Chinese and Western Medicine Therapy

Abstract
Heart failure with preserved ejection fraction(HFpEF)refers to heart failure with a left ventricular ejection fraction of≥50%.It can be triggered by factors such as left ventricular diastolic dysfunction,pulmonary hypertension,systemic inflammatory re-sponse,and activation of the renin-angiotensin-aldosterone system,etc.Clinical treatment primarily involves medications including diu-retics,angiotensin receptor-neprilysin inhibitors(ARNI),sodium-glucose cotransporter 2 inhibitors(SGLT2i),and mineralocorticoid receptor antagonists(MRA),etc.The pathogenesis of HFpEF is characterized by root deficiency and branch excess,with a mixture of deficiency and excess syndromes.Clinically,it often presents as qi deficiency and blood stasis syndrome,yang deficiency with water flood syndrome,and qi-yin deficiency syndrome.Based on different syndrome types,the fundamental treatment principles are supplemen-ting qi and activating blood,as well as nourishing yin and inducing diuresis.Interventions such as Chinese mecicinal compounds,acu-puncture and moxibustion,and acupoint application,etc.are often employed,offering advantages such as definite therapeutic efficacy and fewer adverse reactions.Integrated traditional Chinese and Western medicine in treating HFpEF can enhance clinical efficacy,reduce the incidence of adverse reactions,and provide more options for treatment.However,current treatments for HFpEF still have the follow-ing limitations:(1)Western medicine lacks specific treatment regimens and is prone to causing adverse reactions;(2)The mechanisms of action of TCM treatments remain unclear;(3)Research on integrated treatment has limited sample sizes,and the level of evidence needs improving.Future research should focus on optimizing Western medicine treatment protocols to achieve precision and enhance the monitoring of adverse reactions.It should also make full use of modern pharmacological research methods to explore the mechanisms of TCM formulas.Furthermore,efforts should be made to optimize integrated treatment strategies and develop standardized clinical guide-lines.关键词
