Abstract

Objective:To evaluate the safety of robot-assisted stereotactic hematoma puncture and drainage in the treatment of hypertensive intracerebral hemorrhage(HICH),and to assess its impact on puncture accuracy and complication rates.Methods:35 HICH patients treated with conventional cuboid frame localization at the People's Hospital of Rugao from August 2023 to August 2024 were enrolled and divided into the conventional group,another 35 HICH patients who underwent robot-assisted stereotactic navigation surgery from September 2024 to September 2025 were included into the robot-assisted group.Surgical duration,hematoma drainage time,postoperative recovery time,length of hospital stay,intraoperative blood loss,puncture accuracy,and hematoma puncture satisfaction were compared between the two groups.Postoperative 24-hour hematoma clearance rate,incidence of complication,and long-term outcomes(6-month mortality and disability rates)were also evaluated.Glasgow coma scale(GCS)scores were assessed before surgery,1 day after surgery and 3 days after surgery.Results:Compared to the conventional group,the robot-assisted group showed significantly shorter surgical duration,hematoma drainage time,postoperative recovery time,and length of hospital stay,along with reduced intraoperative blood loss(P<0.05).Puncture accuracy,hematoma puncture satisfaction,and 24-hour hematoma clearance rate were significantly higher in the robot-assisted group than the conventional group(P<0.05).GCS scores on postoperative day 1 and day 3 were significantly higher in the robot-assisted group than the conventional group(P<0.05).The total complication rate was significantly lower in the robot-assisted group than the conventional group(5.71%vs.22.86%,P<0.05).Although the robot-assisted group showed higher survival rate and lower disability rate at 6 months after surgery than the conventional group,the differences were not statistically significant(P>0.05).Conclusion:Robot-assisted stereotactic hematoma puncture and drainage is a safe and effective approach for HICH treatment.It can significantly improve puncture accuracy and patient outcomes,thereby enhancing treatment safety.
