老年晚期重度症状性膝骨关节炎针刀与针刺临床疗效对比
Comparison of the Clinical Efficacy of Acupotomy and Acupuncture in Elderly Patients With Advanced-Stage,Severe Symptomatic Knee Osteoarthritis

Abstract
Objective To compare the efficacy and safety of acupotomy versus filiform-needle acupuncture in elderly patients with advanced-stage,severe symptomatic cold-dampness-obstruction type knee osteoarthritis(KOA).Methods A total of 64 elderly outpatients with severe cold-dampness-obstruction type KOA who refused surgery between January 2023 and January 2025 were enrolled.On the basis of standard care,patients were randomly assigned to a control group(filiform-needle acupuncture,n=31)or an intervention group(acupotomy,n=32).The KOOS pain subscale score,30-second chair-stand test,Barthel Index,and adverse events were assessed at baseline,4 weeks after treatment,and 6 months after treatment.Results Repeat-ed-measures analysis of variance showed that the time×group interaction was statistically significant for all three outcomes(all P<0.001).Simple-effects analyses indicated that the intervention group had better KOOS pain scores and higher 30-second chair-stand counts than the control group at both 4 weeks and 6 months after treatment(all P<0.05).The Barthel Index at 6 months was also significantly higher in the intervention group than in the control group(P<0.001).Conclusion Acupotomy can effectively im-prove joint function,pain severity,and activities of daily living in elderly patients with advanced-stage,severe symp-tomatic cold-dampness-obstruction type KOA,demonstrat-ing superior efficacy to filiform-needle acupuncture.It may serve as an effective non-surgical alternative for patients unwilling to undergo surgery.关键词
