孕早期载脂蛋白B/A1比值对妊娠糖尿病孕妇母婴结局的预测价值
Predictive value of early pregnancy apolipoprotein B/A1 ratio for maternal and neonatal out-comes in patients with gestational diabetes mellitus

Abstract
Aim To investigate the predictive value of the early pregnancy apolipoprotein B/A1 ratio(ApoB/ApoA1)for maternal and neonatal outcomes in patients with gestational diabetes mellitus(GDM).Methods Ninety-two patients with GDM were enrolled as the study group and further divided into a good-outcome group(n=65)and a poor-outcome group(n=27)based on maternal and neonatal outcomes.Eighty healthy pregnant women during the same period were selected as the control group.ApoB and ApoA1 levels were measured in early pregnancy using immunoturbidimetry,and the ApoB/ApoA1 ratio was calculated.Multiva-riate Logistic regression was performed to identify factors influencing maternal and neonatal outcomes.The predictive value of each in-dicator for maternal and neonatal outcomes was evaluated using ROC curve analysis.Results Early pregnancy ApoB and ApoB/ApoA1 were higher in the study group than those in the control group,and higher in the poor-outcome group than those in the good-out-come group.ApoA1 was lower in the study group than that in the control group,and lower in the poor-outcome group than that in the good-outcome group(P<0.05).HOMA-IR was higher in the poor-outcome group than that in the good-outcome group(P<0.05).HOMA-IR≥3.11,ApoB≥0.89 g/L,ApoA1≤0.95 g/L,and ApoB/ApoA1≥0.97 were identified as independent influencing factors for maternal and neonatal outcomes in GDM patients(P<0.05).Early pregnancy ApoB and ApoA1 demonstrated certain predictive efficacy for maternal and neonatal outcomes in GDM patients,and the combined predictive efficacy of ApoB/ApoA1 was superior to that of either indicator alone(P<0.05).Conclusion The early pregnancy ApoB/ApoA1 ratio is highly increased in GDM patients with poor maternal and neonatal outcomes and shows high predictive value for these outcomes.关键词
