Comparison of efficacy and safety between Neuroform Atlas and LVIS stent-assisted coil embolization for intracranial ruptured wide-necked aneurysms

Abstract
Objective:To compare the clinical efficacy and safety of Neuroform Atlas stent versus LVIS stent-assisted coil embolization for the treatment of intracranial ruptured wide-necked aneurysms(IWA).Methods:Clinical data of 56 patients with ruptured IWA who underwent stent-assisted coil embolization at Jiexi People's Hospital and Shantou Central Hospital from June 2016 to June 2024 were retrospectively analyzed.The patients were divided into an Atlas group(n=27)and an LVIS group(n=29).The immediate postoperative embolization degree(Raymond classification),perioperative complications,aneurysm occlusion rate at 3-6 months postoperatively,and the incidence of in-stent stenosis were compared between the two groups.Results:Immediately after the procedure,the proportion of Raymond classⅠ(complete occlusion)was higher in the Atlas group than in the LVIS group(81.5%vs.51.7%,P=0.027).There were no statistically significant differences between the groups in the incidence of ischemic complications(7.4%vs.10.3%)or new asymptomatic ischemic lesions postoperatively(34.8%vs.38.1%)(all P>0.05).Follow-up at 3-6 months postoperatively showed that the Raymond classⅠocclusion rate remained higher in the Atlas group than in the LVIS group(92.6%vs.65.5%,P=0.014),while the proportion of neck remnant(Raymond classⅡ)was higher in the LVIS group(31.0%vs.3.7%).No in-stent stenosis occurred in either group.Conclusion:Both Neuroform Atlas and LVIS stents are safe and effective for emergency coil embolization of ruptured wide-necked intracranial aneurysms.Compared with the LVIS stent,the Neuroform Atlas stent may achieve higher rates of immediate and mid-term complete occlusion.关键词
