Abstract
Objective To explore the risk factors for poor prognosis in patients with unresectable local-ly advanced esophageal squamous cell carcinoma(LAESCC)undergoing radical radiotherapy.Methods A to-tal of 114 patients with unresectable LAESCC who underwent radical radiotherapy in Nanyang Central Hospi-tal from January 2020 to January 2022 were selected.Four patients were excluded during treatment,with a fi-nal total of 110 patients enrolled as study subjects.All patients were followed up for three years after the com-pletion of radical radiotherapy.According to the prognosis outcomes,the patients were divided into a poor prognosis group(68 cases)and a good prognosis group(42 cases).The clinical data of the two groups were compared,and a Cox proportional hazards regression model was established to analyze the influencing factors of poor prognosis in these patients.Results Among the 110 patients,42 survived,with an overall survival time ranging from two to thirty-six months and a mean of(18.03±6.39)months.Statistically significant differences were observed in tumor location,gross tumor volume(GTV),radical radiotherapy dose,Naples Prognostic Score(NPS),and Apelin positive expression rate between the poor prognosis group and the good prognosis group(P<0.05).Cox proportional hazards regression analysis showed that tumor location,GTV,radical radiotherapy dose,positive expression rate of Apelin,and NPS were independent influencing factors for poor prognosis in patients with unresectable LAESCC undergoing radical radiotherapy(P<0.05).Conclusion The poor prognosis of patients with unresectable LAESCC undergoing radical radiotherapy is affected by tumor location,GTV,radical radiotherapy dose,positive expression rate of Apelin,and NPS.Clinicians can implement targeted inter-ventions for the above factors to improve the prognosis of patients.关键词
