针灸治疗乳腺癌化疗后骨髓抑制的临床研究

Abstract
Objective To observe the therapeutic effect of acupuncture at Zusanli,Zhongwan,Qihai and Tianshu on myelosuppression after chemotherapy for breast cancer.Methods A total of 38 patients who underwent postoperative chemotherapy for breast cancer at Affiliated Hospital of Yangzhou University from March to June 2025 were selected and before-after study in the same patient was adopted.When myelosuppression occured in the first course of chemotherapy,leucogen tablets or recombinant human granulocyte colony-stimulating factor(rhG-CSF)were given.In the second course of chemotherapy,acupuncture at the above-mentioned acupoints was added on the basis of the first course.One week between two courses of treatment was set as the elution period.Efficacy was assessed by monitoring blood routine test and total rhG-CSF usage.Results There was no statistically significant difference in the baseline hematological parameters of patients before two chemotherapy courses(P>0.05).After the start of chemotherapy,white blood cell count of patients in the second course of treatment was significantly higher than that in the first course from the 6th day(P<0.05),neutrophil count was significantly higher than that in the first course of treatment at all time points(P<0.05),red blood cell count was significantly higher than that in the first course of treatment on the 6th,9th and 12th days(P<0.05),hemoglobin level was significantly higher than that in the first course of treatment on the 6th and 9th days(P<0.05),platelet count was only significantly higher than that of the first course on the 12th day(P<0.05).The frequency and severity of myelosuppression in the second course of treatment were significantly lower than those in the first course(P<0.05),and the total dose of rhG-CSF used was significantly less than that in the first treatment course(P<0.001).Subgroup analysis showed that white blood cell recovery rate of patients with gradeⅠand gradeⅡmyelosuppression in the second course of treatment were significantly higher than those in the first course(log-rank P<0.001).Conclusion Acupuncture at Zusanli,Zhongwan,Qihai and Tianshu can relieve myelosuppression caused by chemotherapy,reduce rhG-CSF dependence and promote hematological recovery.This regimen is safe and practical and worthy of promotion.关键词
