急性肝硬化并上消化道出血急救预后的危险因素研究
Study on Risk Factors for Emergency Prognosis of Acute Liver Cirrhosis with Upper Gastrointestinal Bleeding

Abstract
Objective To preliminarily explore the risk factors for emergency prognosis in patients with acute liver cirrhosis and upper gastrointestinal bleeding.Methods Clinical data of 80 patients with acute liver cirrhosis and upper gastrointestinal bleeding admitted to Mengchao Hepatobiliary Hospital,Fujian Medical University from April 2024 to March 2025 were retrospectively analyzed.Based on prognosis,patients were divided into a survival group(n=62)and a death group(n=18).Basic information,disease indicators,laboratory test results,imaging findings,shock index,and type of cirrhosis were compared between the two groups.Univariate and multivariate analyses were conducted to screen for risk factors affecting emergency prognosis.Results Univariate analysis revealed significant differences between the two groups in terms of age,gender,Child-Pugh classification of liver function,amount of ascites,volume of bleeding,inci-dence of hepatic encephalopathy,re-bleeding within 3 days after treatment,shock index,diameter of the left gastric vein,and presence of esophagogastric varices(P<0.05).Multivariate logistic regression analysis identified Child-Pugh class C liver function,ascites>3000 mL,shock index>1.5,presence of esophagogastric varices,bleeding volume≥500 mL,re-bleeding within 3 days after treatment,and history of hepatic encephalopathy as independent risk factors affecting the emergency prognosis of acute liver cirrhosis with upper gastrointestinal bleeding.Conclusion Early identification and active intervention targeting independent risk factors such as severe liver dysfunction,shock,massive bleeding,large-vol-ume ascites,varices,and re-bleeding may help improve patient prognosis.关键词
