"空鼓室"技术在内镜下鼓室成形术中的应用
Application of the"empty tympanic chamber"technique in endoscopic tympanoplasty

Abstract
Objective To investigate the value of clinical application of the"empty tympanic cavity"technique in endoscopic inlay tympanoplasty.Methods A retrospective analysis was conducted on 64 patients(64 ears)diagnosed with chronic suppurative otitis media(inactive stage)at Xinhua Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from January 2023 to January 2024.The patients received endoscopic tympanoplasty with either the empty tympanic cavity inlay technique(34 cases,34 ears)or conventional tympanic packing inlay technique(30 cases,30 ears).Both groups underwent tympanoplasty.Pre-and postoperatively(6 months)endoscopic examination and pure-tone audiometry were performed to compare average air conduction pure-tone threshold,air-bone gap,tympanic membrane healing,and ear fullness sensation between the two groups.Results The rate of tympanic membrane healing was 100%in the empty tympanic cavity technique group and 93.3%in the conventional packing group.The postoperative mean air conduction PTA and ABG improved in both groups compared to their respective preoperative values(P<0.05),the air-bone gap difference in the empty group was lower than that in the packed group(P<0.05).The postoperative visual analogue scale score for ear fullness sensation improved more in the empty tympanic cavity technique group compared to the conventional packing group(P<0.05),with no tinnitus observed during the follow-up,while 3 cases in the conventional packing group experienced tinnitus and dizziness.Conclusions In tympanoplasty,the"empty tympanic cavity"inlay technique does not compromise surgical efficacy,but facilitates hearing recovery and improves surgical comfort for patients.This technique is achievable under both endoscopy and microscopy and is worthy of clinical adoption.关键词
