老年性聋伴认知能力下降的影响因素分析
Factors associated with cognitive decline in elderly individuals with age-related hearing loss

Abstract
OBJECTIVE To investigate the primary factors influencing cognitive abilities in patients with age-related hearing loss,providing methodological evidence for preventing and intervening in cognitive decline among this population.METHODS Ninety-eight patients diagnosed with age-related hearing loss were enrolled.Cognitive abilities were assessed using the mini-mental state examination(MMSE).Correlations between auditory-verbal abilities,olfactory function,and cognitive abilities were analyzed.RESULTS 1.Significant differences were observed between the cognitively normal group and the cognitive impairment group in educational attainment,age,gender,olfactory function,orientation,attention,memory,recall ability,and language ability(P<0.05).2.PTA exerts a significant independent effect across all cognitive domains,while SNR50 also shows a significant impact on attention and orientation.When WRS≤80%,cognitive functions in patients with age-related hearing loss were significantly reduced(P<0.05).Cognitive function also declined when SNR50≥7 dB.3.Cognitive function in all subjects showed a significant positive correlation with speech recognition rate in quiet(r=0.342,P<0.001).ROC curves indicated that PTA and WRS were more sensitive predictors of cognitive decline in presbycusis patients.4.Patients with concomitant olfactory dysfunction exhibited a higher likelihood of cognitive impairment.More severe hearing loss and more pronounced olfactory dysfunction correlated with greater cognitive impairment.CONCLUSION Cognitive function in elderly patients with age-related hearing loss is influenced by multiple factors,among which auditory-verbal function and olfactory function are significantly associated with cognitive decline.关键词
