三叉神经痛患者经皮球囊压迫术后效果的影响因素及其预测价值
Influencing factors for postoperative outcomes and predictive value in patients with trigeminal neuralgia undergoing percutaneous balloon compression

Abstract
Objective To investigate the influencing factors for postoperative outcomes of percutaneous balloon compression(PBC)in patients with trigeminal neuralgia(TN),and to construct a predictive model and evaluate its clinical value.Methods A retrospective analysis was conducted on the clinical data of 258 TN patients treated in the Department of Neurosurgery,Meizhou People's Hospital from August 2019 to June 2024.Patients were divided into good outcome group(numeric rating scale(NRS)score 0-1,n=171)and poor outcome group(NRS score 2-3,n=87)according to postoperative NRS scores.Data including age,gender,BMI,disease duration,lesion location,preoperative NRS score,vascular compression status,and intraoperative balloon parameters were collected.Univariate analysis and multivariate Logistic regression analysis were used to identify influencing factors and establish a predictive model.ROC curve was used to evaluate the predictive value of the model.Results Univariate analysis showed that gender,disease duration,hypertension,right-sided vascular compression,preoperative NRS score,and intraoperative balloon filling volume were associated with surgical outcomes(P<0.1).Multivariate Logistic regression analysis revealed that high preoperative NRS score,right-sided vascular compression,and hypertension were independent risk factors for poor surgical outcomes,while large intraoperative balloon filling volume was a protective factor.The AUC of the predictive model was 0.781(95%CI:0.721-0.841),with the sensitivity of 72.37%and the specificity of 76.56%.Conclusion Preoperative NRS score,right-sided vascular compression status,hypertension,and intraoperative balloon filling volume are independent factors affecting postoperative outcomes of PBC in TN patients.The predictive model based on these factors has good predictive value and can provide reference for clinical decision-making.关键词
