SUA/Scr和HbA1c与早期糖尿病肾病发生的关系
Association of serum uric acid/serum creatinine ratio and glycosylated hemoglobin with the progression of early-stage diabetic kidney disease

Abstract
Objective To investigate the association of serum uric acid(SUA)/serum creatinine(Scr)ratio and glycosyla-ted hemoglobin(HbA1c)with the progression of early-stage diabetic kidney disease(DKD).Methods A total of 180 patients with DKD who were diagnosed and treated in The People's Hospital of Xinjiang Uygur Autonomous Region from January 2022 to January 2024 were enrolled as DKD group,and 180 patients with type 2 diabetes mellitus(T2DM)alone were enrolled as control group.The two groups were compared in terms of clinical features,SUA/Scr ratio,and HbA1c.The risk factors for DKD were analyzed,and the value of SUA/Scr ratio and HbA1c in predicting DKD was analyzed.Results Compared with the control group,the DKD group had a significantly longer course of disease,a significantly higher proportion of patients with hypertension,and significantly higher levels of uric acid,fasting blood glucose,postprandial blood glucose,triglycerides,fibrinogen,SUA/Scr ratio,and HbA1c(t=3.021-12.637,x2=14.464,P<0.05).SUA/Scr ratio was positively correlated with HbA1c in DKD pa-tients(r=0.35,P<0.05).Long disease course(OR=1.162,P=0.002),comorbidity with hypertension(OR=2.118,P=0.007),high SUA/Scr ratio(OR=1.629,P=0.018),and high HbA1c(OR=4.892,P<0.001)were independent risk factors for early-stage DKD in T2DM patients.SUA/Scr ratio and HbA1c used alone or in combination had an area under the ROC curve of 0.672,0.838,and 0.850,respectively,in predicting early-stage DKD in T2DM patients.Conclusion High SUA/Scr ratio and elevated HbA1c are risk factors for early-stage DKD in T2DM patients,and the combination of these two indicators have a certain value in predicting early-stage DKD in T2DM patients.关键词
