肝癌伴微血管侵犯患者术后的无瘤生存风险因素分析
Analysis of risk factors for tumor-free survival in patients with liver cancer accompanied by microvascular invasion after surgery

Abstract
Objective:To explore the relationship between the changes in systemic immune inflammatory index(SII),systemic inflammatory response index(SIRI),and albumin-bilirubin(ALBI)grade after precise liver resection in patients with solitary liver cancer accompanied by microvascular invasion and tumor-free survival.Methods:A retrospective analysis of 100 patients with single hepatocellular carcinoma accompanied by microvascular invasion who underwent precise liver resection in our hospital from January 2017 to December 2020 was conducted as the research subjects.According to the follow-up situation,they were divided into tumor-free survival<5 years and tumor-free survival≥5 years.The area under the ROC curve predicted the diagnostic value of SII,SIRI,and ALBI grading for tumor-free survival.Cox regression analysis was used to determine the influencing factors of tumor-free survival in patients with single hepatocellular carcinoma accompanied by microvascular invasion.Results:The proportion of PLT,NEU,AMC,GGT,SII,SIRI,and ALBI grades 2-3 in patients with tumor free survival<5 years was significantly higher than that in patients with tumor free survival≥5 years(P<0.05);The area under the curve for predicting tumor free survival using SII,SIRI,ALBI grading,and combined diagnosis is 0.771(95%CI=0.658-0.884,P<0.001)、0.817(95%CI=0.713-0.922,P<0.001)、0.833(95%CI=0.738-0.929,P<0.001)、0.887(95%CI=0.810-0.964,P<0.001);PLT,NEU,AMC,GGT,SII,SIRI,and ALBI grading are independent factors affecting the tumor free survival of patients with single liver cancer and microvascular invasion after precision liver resection(P<0.05).Conclusion:In patients with solitary liver cancer accompanied by microvascular invasion,the SII,SIRI,and ALBI grading after precise liver resection are associated with tumor-free survival.关键词
