Relationship between lipid levels and the efficacy of continuous blood purification in the treatment of elderly patients with end-stage heart failure

Abstract
Objective To investigate the relationship between lipid levels and the efficacy of continuous blood purification(CRRT)in the treatment of elderly patients with end-stage heart failure(ESHF).Methods Relevant data of 206 elderly patients with ESHF admitted to our hospital from May 2022 to May 2025 were retrospectively collected and analyzed.All patients received bedside CRRT.According to treatment efficacy,patients were assigned to an effective group(n=166)and an ineffective group(n=40).Lipid levels(high-density lipoprotein cholesterol[HDL-C],triglycerides[TG],low-density lipoprotein cholesterol[LDL-C],total cholesterol[TC])before treatment and clinical data were compared between the two groups.Logistic regression analysis was used to analyze the factors influencing the efficacy of CRRT in the treatment of eld-erly ESHF,and receiver operating characteristic(ROC)curve analysis was used to evaluate the predictive value of lipid lev-els for the efficacy of CRRT.Results Compared with the effective group,the ineffective group had lower pre-treatment HDL-C levels,higher TG,LDL-C,and TC levels,older age,a higher proportion of patients with NYHA class Ⅲ-Ⅳ,and a higher proportion of patients with a history of coronary heart disease,with statistically significant differences(P<0.05).After adjusting for confounding factors such as age and NYHA class in the multivariate logistic regression model,lipid pa-rameters(HDL-C,TG,LDL-C,TC)remained independent influencing factors for CRRT efficacy(P<0.05).ROC curve analysis showed that the combined model based on lipid levels had an area under the curve of 0.918,with a sensitivity of 0.900 and specificity of 0.801 for predicting the efficacy of CRRT in elderly ESHF patients.Conclusion Lipid levels inde-pendently influence the efficacy of CRRT in the treatment of elderly ESHF,and the combined model based on lipid levels has good predictive value for the efficacy of CRRT in these patients.关键词
