Clinical Efficacy of Scarification Therapy Combined with Puding Mixture in Treating Moderate-to-Severe Rosacea Observed via Multi-View 3D Nasal Reconstruction

Abstract
Objective To evaluate the clinical efficacy and morphological changes of hyperplastic tissue excision and epidermal scarification(referred to as scarification therapy)integrated with the heat-clearing Chinese herbal formula Puding Mixture in treating moderate-to-severe rosacea through a controlled trial combined with three-dimensional(3D)reconstruction technology.Methods Forty-two patients with moderate-to-severe rosacea treated at the Dermatology Outpatient Clinic of Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from August 2023 to November 2024 were selected.Patients were randomly divided into a trial group and a control group using a random number table,with 21 patients in each group.The trial group received hyperplastic tissue excision and reticular scarification using a five-bladed knife,combined with postoperative oral administration of Puding Mixture.The control group was treated with an UltraPulse super-pulsed CO2 laser therapy.The treatment course was 6 weeks,with efficacy evaluated after 3 months of treatment.Efficacy was assessed through clinical nasal scores(erythema,telangiectasia,sebum secretion,skin thickening),Dermatology Life Quality Index(DLQI)scores,and 3D morphological analysis.Three-dimensional reconstruction was performed using Blender software,and volume calculation was completed using COMSOL Multiphysics.Results(1)Clinical nasal scores:3 months after treatment,scores for nasal erythema,skin thickening,telangiectasia,and sebum secretion were improved in both groups compared to before treatment(P＜0.01).The improvements in skin thickening and sebum secretion scores were significantly greater in the trial group than in the control group,with statistically significant differences(P＜0.05).(2)Quality of life scores:3 months after treatment,DLQI scores decreased in both groups compared to before treatment(P＜0.01),and the decrease was significantly greater in the trial group than in the control group(P＜0.05).(3)3D morphological analysis:3 months after treatment,the percentage reduction in nose size in the trial group was(24.13±14.63)%,significantly higher than(8.28±5.33)%in the control group,with a statistically significant difference between groups(P＜0.05).Conclusion Scarification therapy combined with Puding Mixture demonstrates definite efficacy in treating patients with moderate-to-severe rosacea.It is significantly superior to CO2 laser treatment in improving skin thickening,sebum secretion,and quality of life,and leads to more substantial reduction of nose size.Three-dimensional reconstruction technology provides an objective and quantitative means for efficacy evaluation.Scarification therapy integrates the principles of TCM's needling-scraping technique with modern surgical methods,showing potential for clinical promotion.However,further multicenter studies are warranted to validate its long-term efficacy and underlying mechanisms.关键词
