经鼻蝶窦入路手术中通过蝶窦副分隔定位不明显外侧视神经颈内动脉隐窝的可行性
Feasibility of locating the obscure lateral opticocarotid recess via the ac-cessory sphenoidal septem in endonasal transsphenoidal surgery

Abstract
Aim:To investigate the feasibility of locating the obscure lateral opticocarotid recess(LOCR)via the acces-sory sphenoidal septem(ASS)in endonasal transsphenoidal surgery.Methods:Original imaging data from 132 patients who underwent cranial computed tomography and magnetic resonance angiography in the Department of Neurosurgery,the Second Affiliated Hospital of Zhengzhou University from January 2020 to December 2022 were selected.Three-dimensional recon-struction of the sphenoid sinus and its surrounding structures was performed,and the endoscopic transsphenoidal surgical view was simulated.The three-dimensional coordinates of the LOCR located by the ASS method(the intersection of the ASS ridge or its extension line with the junction line between the roof and lateral wall of the sphenoid sinus)were compared with those located by the optic strut method.Pearson correlation analysis was used to assess the correlation between the coordi-nate values obtained by the 2 methods.Using the optic strut method and neuronavigation results as references,the accuracy and spatial consistency of the ASS method for locating the LOCR were validated in 104 sides of virtual surgeries and 22 sides of actual endoscopic endonasal transsphenoidal surgeries.Results:Among the 264 sides in 132 patients,171 sides(64.77%)had an obscure LOCR,of which104sides(39.39%)had a coexisting ASS.In these104 sides,there were no significant differences in the coordinates of the LOCR in the right(R),anterior(A),and superior(S)directions between the ASS method and the optic strut method(P>0.05),with correlation coefficients of 0.935,0.887,and 0.968,respectively(P<0.05).In the virtual surgical approach,95.19%(99/104)of the LOCR could be located by the ASS method.In the 22 sides of actual surgeries,the location of the obscure LOCR identified by the ASS method was consistent with that deter-mined by neuronavigation.Conclusion:The LOCR has a close anatomical relationship with the ipsilateral ASS ridge or its extension line.In the endonasal transsphenoidal surgical approach,the obscure LOCR can be located via the intersection of the ASS ridge or its extension line with the junction line between the roof and lateral wall of the sphenoid sinus.关键词
