DSC-MRI联合扩散加权成像对脑胶质母细胞瘤和脑转移瘤的鉴别诊断价值
The differential diagnostic value of DSC-MRI combined with diffusion-weighted imaging for glioblastoma and brain metastases

Abstract
Objective To explore the differential diagnostic value of dynamic susceptibility contrast enhanced-magnetic resonance imaging(DSC-MRI)combined with diffusion-weighted imaging(DWI)for glioblastoma(GBM)and brain metastases(BM).Methods From January 1,2020 to December 31,2024,128 patients with brain tumors confirmed by surgery and pathology admitted to the First Hospital of Nanping were enrolled,including 96 patients with GBM and 32 patients with BM.Receiver operating characteristic(ROC)curves were used to analyze the differential diagnostic value of DSC-MRI combined with DWI for GBM and BM.Results There was significant difference in bleeding extent between the GBM group and the BM group(P<0.05).For the BM group,the GBM group had higher peritumoral rCBV and tumor PSR(P<0.05).For the BM group,the GBM group had a lower peritumoral ADC(P<0.05).The combination of peritumoral rCBV,tumor PSR,and peritumoral ADC had a highest AUC of 0.952(95%CI=0.899-0.982)in distinguishing GBM and BM,which was superior to the individual differential diagnoses of peritumoral rCBV,tumor PSR,and peritumoral ADC(Zcombination-peritumoral rCBV=2.123,P=0.034,Zcombination-tumor PSR=4.226,P<0.001,Zcombination-peritumoral ADC=3.144,P=0.002),the highest sensitivity and specificity were 89.58%and 90.62%,respectively.Conclusions DSC-MRI combined with DWI has high differential diagnostic value for GBM and BM.关键词
