黄连解毒汤治疗帕金森病认知功能障碍患者临床研究
Clinical Study on Huanglian Jiedu Decoction(黄连解毒汤)in Treatment of Parkinson's Disease Patients with Cognitive Impairment

Abstract
Objective To explore the effect of Huanglian Jiedu Decoction(黄连解毒汤)on neurotrophin,brain neurotransmit-ters and clinical efficacy in the treatment of Parkinson's disease(PD)patients with cognitive impairment.Method:A total of 120 PD patients with cognitive impairment admitted the hospital from February 2021 to February 2023 were divided into treatment group(60 cases)and control group(60 cases).The control group was treated with western medicine and the treatment group was combined with Huanglian Jiedu Decoction on thee basis of the control group.The treatment period of the two groups lasted for 12 weeks.The clinical efficacy of the two groups after 12 weeks of treatment was compared,and the effects of cognitive function,de-gree of illness and activities of daily living,neurotrophin,brain neurotransmitters and quality of life before and after 12 weeks of treatment were compared.Result The effective rate of the treatment group was higher than that of the control group(P＜0.05).After 12 weeks of treatment,the mini-mental state examination(MMSE)scores and Montreal cognitive assessment(MoCA)scores of the two groups were higher than those before treatment(P＜0.05).The scores of the treatment group were higher than those of the control group(P＜0.05).After 12 weeks of treatment,the unified Parkinson's disease rating scale(UPDRS)scores of the two groups were lower than those before treatment,and the score of the treatment group was lower than that of the control group(P＜0.05).The activities of daily living(ADL)scores were higher than those before treatment(P＜0.05)and the ADL score in the treatment group was higher than that of the control group(P＜0.05).After 12 weeks of treatment,the levels of brain-derived neurotrophic factor(BDNF)and glial cell line derived neurotrophic factor(GDNF)in both groups were higher than those before treatment(P＜0.05).The levels in the treatment group were higher than those in the control group(P＜0.05).After 12 weeks of treatment,the levels of homocysteine(Hey)and acetylcholinesterase(AChE)in both groups were lower than those before treat-ment,and the levels of the treatment group were lower than those of the control group(P＜0.05).The levels of acetylcholine(Ach)in both groups were higher than those before treatment,and the level of the treatment group was higher than that of the con-trol group(P＜0.05).After 12 weeks of treatment,the scores of the Parkinson's disease quality of life questionnaire(PDQ-39)in both groups were lower than those before treatment(P＜0.05).The score of the treatment group was lower than that of the con-trol group(P＜0.05).Conclusion Huanglian Jiedu Decoction can increase the levels of BDNF and GDNF,down-regulate the levels of Hey and AChE,and up-regulate the level of Ach in the PD patients with cognitive impairment.It has significant effects,and can improve cognitive function and quality of life.关键词
