Effect of Cluster Needling at Scalp Acupoints Combined with Huanglian Jiedu Decoction(黄连解毒汤)on Patients with Alzheimer's Disease Based on Brain-Gut Axis Theory

Abstract
Objective To research the effect of cluster needling at scalp acupoints combined with Huanglian Jiedu Decoction(黄连解毒汤)on the patients with Alzheimer's disease(AD)based on brain-gut axis theory.Methods From June 2023 to May 2024,68 patients with AD who admitted in the hospital were selected and randomly divided into cluster needling at scalp acu-points combined with Huanglian Jiedu Decoction group and conventional western medicine group with 34 patients in each group.The patients in the conventional western medicine group only took donepezil hydrochloride tablets,while those in the cluster nee-dling at scalp acupoints combined with Huanglian Jiedu Decoction group were treated with cluster needling at scalp acupoints combined with Huanglian Jiedu Decoction.The scores of mini-mental state examination(MMSE),Montreal cognitive assessment(MoCA)score and activities of daily living(ADL)were compared between the two groups before and after treatment.The number of intestinal flora,the levels of inflammatory factors,serum brain-derived neurotrophic factor(BDNF),amyloid β-protein(Aβ)precursor protein(APP)and amyloid β-protein1-42(Aβ1-42)were compared between the two groups before and after treatment.Result After treatment,the scores of MMSE and MoCA in the cluster needling at scalp acupoints combined with Huanglian Jiedu Decoction group were significantly higher than those of the conventional western medicine group at the same period,and the ADL scores were significantly lower than those of the conventional western medicine group at the same period(P＜0.05).After treat-ment,the scores of TCM syndromes in the cluster needling at scalp acupoints combined with Huanglian Jiedu Decoction group were significantly lower than those in the conventional western medicine group(P＜0.05).After treatment,the numbers of Lacto-bacillus and Bifidobacterium in the cluster needling at scalp acupoints combined with Huanglian Jiedu Decoction group were sig-nificantly higher than those in the conventional western medicine group,while the numbers of Escherichia coli and Enterococcus were significantly lower than those in the conventional western medicine group(P＜0.05).The levels of C-reactive protein(CRP),tumor necrosis factor-alpha(TNF-α)and interleukin-6(IL-6)in the cluster needling at scalp acupoints combined with Huanglian Jiedu Decoction group after treatment were significantly lower than those in the conventional western medicine group at the same period(P＜0.05).The serum BDNF in the cluster needling at scalp acupoints combined with Huanglian Jiedu Decoction group after treatment was significantly higher than that in the conventional western medicine group at the same period,and the levels of APP and Aβ1-42 were significantly lower than those in the conventional western medicine group at the same peri-od(P＜0.05).Conclusion The combined application of cluster needling at scalp acupoints and Huanglian Jiedu Decoction can not only directly regulate the cerebral cortex function of the AD patients,but also effectively regulate their intestinal microecologi-cal environment,providing a comprehensive treatment scheme for AD patients.This therapy significantly improved the intestinal flora balance and neuroinflammatory reaction of the AD patients,effectively blocked the pathological transmission of intestinal-brain axis,and significantly improved the cognitive function of patients.关键词
