食管癌患者围术期家庭照顾者负担体验及需求的纵向质性研究
Burden experience and demands of family caregivers during perioperative period of esophageal cancer:a longitudinal qualitative study

Abstract
Objective To explore the dynamic changes in burden experience and demands of family caregivers of patients with esopha-geal cancer during perioperative period.Methods A prospective longitudinal qualitative study was conducted.The 18 family caregivers of patients with esophageal cancer in perioperative period from the Department of Thoracic Surgery of a tertiary grade-A hospital in An-hui Province were selected as research subjects by purposive sampling.Based on the Timing It Right(TIR),five semi-structured inter-views were conducted with the family caregivers.Combined with synchronic and diachronic analysis strategies,cross-sectional and lon-gitudinal analyses were performed on the interview data.Results Five core themes and seventeen sub-themes were extracted,includ-ing mixed emotions during the diagnosis with the expectation of obtaining treatment information and psychological support;obstacles in the transition of caregiving roles in the stable period leading to caregiving predicaments;transfer of caregiving tasks in the preparation peri-od with prominent demands for health education and dyadic communication;insufficient coping capacity in the adjustment period re-quiring the expansion of external support networks;and active self-adjustment in the adaptation period with an urgent hope for daily life restoration.Conclusion Medical staff should conduct dynamic assessments of burden experience and demands of family caregiv-ers during perioperative period of esophageal cancer,and formulate personalized and whole-cycle intervention strategies.Emotional sup-port should be provided for family caregivers during hospitalization,with the implementation of diversified health education and communi-cation skills training.After discharge,care experience of family caregivers can be improved and their care burden alleviated through mea-sures such as carrying out coping capacity training,enhancing sense of benefit from the disease,and strengthening social support.关键词
