电针"骶四穴"治疗良性前列腺增生伴中重度下尿路症状的临床研究
Clinical efficacy of electroacupuncture at"four acupoints of sacral region"on benign prostatic hyperplasia with moderate and severe lower urinary tract symptoms

Abstract
Objective It is to observe the clinical therapeutic effect of electroacupuncture at"four acupoints of sacral region"on benign prostatic hyperplasia(BPH)with moderate and severe lower urinary tract symptoms(LUTS).Methods A total of 92 patients with BPH and moderate and severe LUTS treated at Huangpu District Xiangshan Traditional Chinese Medicine Hospital from July 2023 to September 2024 were selected and randomly divided into an electroacupuncture group(46 cases,2 cases dropped out,1 case eliminated)and a western medicine group(46 cases,3 cases dropped out,1 case discontinued).Based on lifestyle intervention,the electroacupuncture group was treated with electroacupuncture at"four points of sacral region"(the acupoints at 0.5 cun beside bilateral to the edge of sacrococcygeal joint and bilateral Huiyang)3 times per week,the western medicine group was treated with tamsulosin hydrochloride sustained-release capsule by oral administration once every day,both groups were treated for 8 weeks.The scores of International Prostate Symptom Score(IPSS)(including scores of bladder obstruction symptoms and bladder irritation symptoms),and the score of quality of life(QOL)of patients in the two groups before treatment,after 8 weeks of treatment and at 1 month after treatment,the prosta-tic volume and postvoid residual(PVR)before and after treatment were observed and compared,the therapeutic effects of the two groups were evaluated after 8 weeks of treatment.Results After 8 weeks of treatment,the IPSS scores,bladder ob-struction symptom scores,bladder irritation symptom scores and QOL scores were significantly lower than those before treat-ment in both groups(all P<0.05),and the scores of IPSS,scores of straining to urinate,scores of difficulty urinating,total scores of bladder obstruction symptom in the electroacupuncture group were significantly lower than those in the west-ern medicine group(all P<0.05),there were no significant differences in the other scores of bladder obstruction and bladder irritation symptom scores and QOL scores between the two groups(all P>0.05).At1 month after treatment,the IPSS scores and QOL scores were significantly lower than those before treatment in both groups(all P<0.05),but the IP-SS scores were higher than those after treatment(all P<0.05),and the IPSS scores in the electroacupuncture group were significantly lower than those in the western medicine group(all P<0.05).There were no statistically significant differ-ences in prostate volume and PVR between or within the two groups before and after treatment(all P>0.05).The overall effective rate of the electroacupuncture group was significantly higher than that of the western medicine group[86.05%(37/43)vs 80.95%(34/42),P<0.05].Conclusion Electroacupuncture at"four acupoints of sacral region"and oral administration of tamsulosin can alleviate moderate to severe lower urinary tract symptoms in BPH patients and improve their quality of life,but have little effect on prostate volume and PVR.Electroacupuncture at"four acupoints of sacral region"has better therapeutic effects and is more significantly in improving symptoms of bladder obstruction.关键词
