Effects of acupoint injection at Zusanli acupoint with astragalus injection on diaphragmatic function and weaning in patients treated with mechanical ventilation

Abstract
Objective It is to investigate the effects of acupoint injection at Zusanli acupoint with astragalus injection on diaphragmatic function and weaning in patients treated with mechanical ventilation.Methods A total of 112 patients with severe pneumonia admitted to the Department of ICU of Shanghai Baoshan District Hospital of Integrated Traditional and Western Medicine from January 2021 to December 2023 and suffered from diaphragmatic dysfunction during mechanical ventilation were enrolled,and were randomly divided into experimental group and control group by random number table method,with 56 patients in each group.The control group was treated with invasive mechanical ventilation combined with comprehensive medical therapy,while the experimental group was treated with acupoint injection at Zusanli acupoint with astragalus injection on the basis of therapy in the control group,both groups were treated for 7 days.The diaphragmatic thickening fraction(DTF),diaphragmatic excursion(DE),oxygenation index(OI),respiratory shallow breathing index(RSBI)and lung compliance were compared between the two groups before treatment and after 7 days of treatment.Addi-tionally,the duration of mechanical ventilation,the successful rate of weaning on the 7th day and the incidence of adverse reactions in both groups were calculated.Results After 7 days of treatment,the DTF,DE,OI and lung compliance in both groups were significantly higher than those before treatment(all P<0.05),and all these indicators in the experimental group were significantly higher than those in the control group(all P<0.05);the RSBI in both groups were significantly lower than those before treatment(both P<0.05),and the RSBI in the experimental group was significantly lower than that in the control group(P<0.05).The duration of mechanical ventilation in the experimental group was significantly shorter,and the successful rate of weaning on the 7th day was significantly higher than those in the control group(both P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Acupoint injection at Zusanli acupoint with astragalus injection can enhance diaphragmatic function,shorten the duration of mechanical ventilation and improve respiratory function of patients treated with mechanical ventila-tion,and can increase the success rate of weaning with good safety.关键词
