Abstract
Objective To investigate the effects of non-invasive ventilation(NIV)combined with high-flow nasal cannula oxygen therapy(HFNC)on respiratory and oxygenation functions of patients with acute respiratory failure.Methods A total of 77 patients with a-cute respiratory failure admitted to our hospital were selected and assigned to groups using a random number table.The NIV group(38 pa-tients)received ventilatory support via a non-invasive ventilator,while the HFNC group(39 patients)received a combination of non-inva-sive ventilatory support and HFNC therapy.Both groups were monitored until discharge and followed up for 28 days,during which various clinical indicators were statistically compared.Results Following treatment,the HFNC group demonstrated shorter ICU lengths of stay,as well as shorter durations for the resolution of cyanosis and shortness of breath,compared to the NIV group.In both groups,Borg dyspnea scores decreased and Oxygenation Indices(OI)increased,with the improvements being more significant in the HFNC group.Both groups exhibited increases in SpO2 and PaO2,along with decreases in PaCO2;notably,the HFNC group demonstrated superior blood gas parameters.Further-more,levels of CRP,IL-6,and WBC decreased in both groups,with the HFNC group achieving lower levels.The HFNC group also recorded a greater number of days of survival free from mechanical ventilation within the 28-day follow-up period compared to the NIV group(all P<0.05).Although the 28-day mortality rate in the HFNC group was lower than that in the NIV group,the difference was not statistically signif-icant(P>0.05).Conclusion In patients with acute respiratory failure,the combination of NIV and HFNC is more effective in reducing treatment-related duration,improving respiratory and oxygenation functions,attenuating inflammatory responses,and extending the duration of survival free from mechanical ventilation within 28 days;its overall therapeutic efficacy is superior to that of NIV alone.关键词
