止痛利尿艾盐包热熨联合通泉散穴位贴敷预防初产妇产后尿潴留临床研究
Clinical Study on Pain-Relieving and Diuretic Moxa Salt Pack Hot Compress Combined with Tongquan Powder Acupoint Application for Preventing Postpartum Urinary Retention in Primiparas

Abstract
Objective:To observe the clinical efficacy of pain-relieving and diuretic moxa salt pack hot compress combined with Tongquan powder acupoint application on the basis of routine postpartum interventions for preventing postpartum urinary retention in primiparas.Methods:A total of 98 primiparas who underwent vaginal delivery at Shengzhou Maternity and Child Healthcare Hospital from March 2022 to November 2024 were enrolled and randomly divided into a control group and an observation group using a random envelope method,with 49 cases in each group.Both groups received routine postpartum care and symptomatic medication.The control group was treated with Tongquan powder acupoint application,while the observation group was treated with Tongquan powder acupoint application combined with pain-relieving and diuretic moxa salt pack hot compress.Both groups received intervention for three days.The clinical efficacy,incidence of urinary retention,and incidence of adverse reactions were compared between the two groups.The time to first voiding,lochia volume,post-void residual bladder volume,uterine fundal height,bladder function indicators(bladder first sensation volume,maximum urinary flow rate,detrusor pressure at maximum urinary flow rate),and uterine contraction pain score[Numerical Rating Scale(NRS)score]were recorded and measured.Results:After intervention,the total effective rate in the observation group was higher than that in the control group(P＜0.05).The observation group showed earlier time to first voiding,less post-void residual bladder volume,and lower incidence of urinary retention compared to the control group,with differences being significant(P＜0.05).The observation group had higher bladder first sensation volume,maximum urinary flow rate,and detrusor pressure at maximum urinary flow rate than the control group(P＜0.05).Both groups showed decreased lochia volume and NRS scores on the 3rd day postpartum compared to the 1st day postpartum(P＜0.05).On the 3rd day postpartum,the observation group had less lochia volume,lower NRS scores,and greater reduction in uterine fundal height compared to the control group,with differences being significant(P＜0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P＞0.05).Conclusion:The application of pain-relieving and diuretic moxa salt pack hot compress combined with Tongquan powder acupoint application on the basis of routine postpartum care and symptomatic medication can improve bladder function in primiparas,promote uterine involution,and effectively prevent postpartum urinary retention.关键词
