基于清浊相干理论探析针刺治疗痛风性肾病临证思路
Analysis of Clinical Strategies for Acupuncture Treatment of Gouty Nephropathy Based on the Clear-Turbid Interference Theory
摘要
Abstract
The concept of"clear-turbid interference"is an ancient Chinese medical summary of pathological changes in which the metabolism of clear and turbid substances becomes disordered,leading to dysfunction in the qi movement.From the perspective of"clear-turbid interference,"the treatment of gouty nephropathy is considered to stem from the basic pathogenesis of impaired spleen and kidney function,resulting in disordered metabolism of clear and turbid substances.The pathological products include dampness,turbidity,phlegm,and stasis,which are categorized as turbid pathogenic factors.Based on the holistic view and syndrome differentiation principles of traditional Chinese medicine,the treatment emphasizes the therapeutic principles of lifting the clear and lowering the turbid and regulating the qi movement.Acupuncture focuses on key points:Zhongwan(CV12)point,Tianshu(ST25)point,Guanyuan(CV4)point,and Zhongji(CV3)point to tonify the spleen and kidney and lift the clear;Dadu(SP2)point,Taibai(SP3)point,Rangu(KI2)point,and Taixi(KI3)point to clear damp-heat and lower the turbid;and Xuehai(SP10)point,Geshu(BL17)point,and Ashi points to assist in the ascending and descending functions.This comprehensive approach addresses both the root cause and the symptoms,combining tonification and elimination methods,thereby promoting the ascent of clear yang,descent of turbid yin,and overall regulation of qi movement.关键词
