Abstract
Objective To explore the application value of combined transabdominal sonography(TAS)and transvaginal sonography(TVS)in the diagnosis of ectopic pregnancy(EP),so as to provide a reference for precise clinical diagnosis and treatment.Methods The clinical data of 150 patients with acute gynecological and obstetric abdominal pain admitted to our hospital from October 2024 to October 2025 were retrospectively analyzed.All patients underwent both TAS and TVS examinations.Using surgical findings as the diagnostic"gold standard",the detection rates of TAS,TVS alone,and their combination for different types of EP were compared and analyzed.The clinical diagnostic value of the two modalities and their consistency with surgical results were evaluated.Results Among the 150 patients,108 cases were confirmed to have EP postoperatively.TAS alone detected 88 cases,TVS alone detected 97 cases,and the combination detected all 108 cases.No statistically significant difference was found in the detection rates for abortive-type EP among the different ultrasound methods(P>0.05).The combined examination demonstrated higher detection rates for both unruptured and ruptured types of EP compared to TAS or TVS alone(P<0.05).The diagnostic efficacy of combined examination was superior to that of single-item examination(P<0.05).Furthermore,the sensitivity and accuracy of TVS in diagnosing EP were higher than those of TAS(P<0.05).Consistency analysis revealed that the combined examination had excellent agreement with surgical results(Kappa=0.901,P=0.000),TVS showed moderate agreement(Kappa=0.617,P=0.000),and TAS showed poor agreement(Kappa=0.365,P=0.000).Conclusions The combination of TAS and TVS demonstrates higher diagnostic value for EP,improving the detection rate across different EP types and showing strong consistency with surgical findings,which provides important guidance for developing individualized clinical treatment plans and holds significant value for widespread clinical application.关键词
