Abstract
Objective To investigate the effects of 3.0T magnetic resonance imaging(MRI)dynamic contrast-enhanced scanning on the demonstration rate of fistulas,diagnostic score of internal opening,and diag-nostic accuracy of different classifications in complex anal fistula.Methods Total 70 patients diagnosed with complex anal fistula in our hospital from January 2023 to December 2025 who underwent 3.0T MRI T1WI+T2WI,diffusion-weighted imaging(DWI),dynamic contrast-enhanced scanning and surgical treatment were enrolled.Taking surgical and pathological results as the gold standard,the demonstration rates of internal open-ing,main fistula and branch fistula,diagnostic score of internal opening,and diagnostic accuracy of different Parks classifications were compared between two sequence combinations(T1WI+T2WI+DWI vs.T1WI+T2WI+DWI+dynamic contrast-enhanced).Results The demonstration rates of internal opening,main fistula and branch fistula by dynamic contrast-enhanced sequence were 81.45%,86.61%and 85.19%respec-tively,which were significantly higher than 65.32%,74.11%and 44.44%of T1WI+T2WI+DWI sequence(all P<0.05).The diagnostic score of internal opening of the dynamic contrast-enhanced sequence was higher than that of T1WI+T2WI+DWI sequence.The diagnostic accuracies for trans-sphincteric,supra-sphincteric and extra-sphincteric types were 94.29%,90.48%and 100.00%respectively,all higher than 77.14%,61.90%and 64.29%of T1WI+T2WI+DWI sequence(P<0.05).Conclusion 3.0T MRI dynamic contrast-enhanced scanning can significantly improve the demonstration rate of lesion structures and the diagnos-tic accuracy of classification in complex anal fistula,providing reliable evidence for precise preoperative evaluation.关键词
