血清eCIRP对老年脓毒症的诊断价值研究
Diagnostic value of serum eCIRP in sepsis in elderly patients

Abstract
Objective To explore the diagnostic value of serum extracellular cold-inducible RNA-binding protein(eCIRP)in sepsis in elderly patients.Methods Elderly patients(aged≥60 years old)with infectious diseases hospitalized in Sichuan Provincial People's Hospital from August 1 to December 31,2022 were selected for cross-sectional survey,and elderly patients without infection were included as the non-infection group.According to the diagnostic criteria for sepsis in"International consensus on the definition of sepsis and septic shock"(2016),infected patients were divided into a sepsis group and a non-sepsis group.Clinical data and blood specimens of patients were collected. Serum eCIRP concentrations of three groups of patients were compared. Diagnostic value of eCIRP for sepsis in elderly patients was evaluated with the area under the receiver operating characteristic curve (AUC). AUCs of inflammatory indicators were compared with Delong's test. Results A total of 109 elderly patients with infectious diseases (44 in the sepsis group, 65 in the non-sepsis group) and 42 patients in non-infection group were included in analysis. Compared with non-infection group, serum eCIRP concentrations in both the non-sepsis group and sepsis group increased (both P<0.001). Compared with the non-sepsis group, serum eCIRP con-centration in sepsis group increased (P <0.001 ). The ACU of eCIRP combined with neutrophil percentage (NEUT％), C-reactive protein (CRP), and procalcitonin (PCT) detection for the diagnosis of sepsis in elderly patients was 0.963, which was higher than the single indicators of eCIRP (AUC=0.862), NEUT％ (AUC= 0.855), CRP (AUC=0.877), and PCT (AUC=0.845), differences were all statistically significant (all P <0.05).Conclusion eCIRP has diagnostic value for sepsis in elderly patients and is an important biomarker for diag-nosis of sepsis.The combined detection of eCIRP with NEUT％,CRP,and PCT can significantly improve the ac-curacy in diagnosing sepsis in elderly patients.关键词
