体外冲击波疗法联合单髁置换术治疗膝关节内侧间室骨性关节炎的效果
Outcome of extracorporeal shock wave therapy combined with unicompartmental knee arthroplasty in treating medial compartment knee osteoarthritis

Abstract
Objective To assess the outcome of extracorporeal shock wave therapy(ESWT)combined with unicompartmental knee arthroplasty(UKA)in the treatment of patients with medial compartment knee osteoarthritis.Methods The clinical data of 68 patients with medial compartment knee osteoarthritis who were treated at our hospital from May 2022 to May 2024 were collected.The patients were divided into the control group(received UKA alone)and the observation group(received UKA combined with ESWT)according to the different treatments,with 34 cases in each group.The two groups were compared in terms of the American Knee Society Score(KSS),knee range of motion(ROM),serum levels of inflammatory and pain-related factors[interleukin(IL)-1β,IL-6,tumor necrosis factor-alpha(TNF-α),and prostaglandin E2(PGE2)],and the World Health Organization Quality of Life-Brief Version(WHOQOL-BREF)scores.The occurrence of complications within 3 months after surgery was recorded in both groups.Results The postoperative KSS scores and knee ROM in both groups gradually increased over time,with statistically significant differences(P<0.05).At 1 month and 3 months after surgery,the knee scores and function scores in the observation group were higher than those in the control group,and the knee ROM was greater than that in the control group,with statistically significant differences(P<0.05).The levels of serum inflammatory and pain-related factors in both groups gradually decreased after surgery(P<0.05).At 1 month and 3 months after surgery,the levels of IL-1β,IL-6,TNF-α,and PGE2 in the observation group were significantly lower than those in the control group(P<0.05).At 3 months after surgery,all domain scores of the WHOQOL-BREF in both groups were significantly higher than those before surgery(P<0.05),and all domain scores of the WHOQOL-BREF in the observation group were significantly higher than those in the control group(P<0.05).The incidence of complications 3 months after surgery in the observation group was significantly lower than that in the control group(P<0.05).Conclusion The combination of UKA and ESWT can significantly promote the recovery of knee function in patients with medial compartment knee osteoarthritis,alleviate pain,reduce levels of serum inflammatory factors,and decrease the incidence of postoperative complications,thereby significantly improving quality of life.关键词
