农村慢性病网格化高质量防治体系构建
Establishment of a high-quality grid management system for chronic diseases of rural residents:Take County T as an example

Abstract
Establishment of a high-quality grid management system for chronic diseases of rural residents requires a coherent policy design,a four-level collaboration model,and a trinity philosophy for prevention and con-trol.However,there are challenges for establishing such an integrated system,including poor policy coherence across different policy schemes,delayed development of the prevention and control platform,and low public aware-ness of rural residents.To address these issues,the paper takes T County in Anhui Province as an example,draws on social welfare policy theory,and summarizes practical experiences of grid-based chronic disease prevention in rural areas.It argues that it is significant to spare great efforts focusing on enhancing policy integration,accelera-ting platform development,and strengthening health awareness of rural residents.While the institutional founda-tions in such an integrated policy system must be consolidated,collaborative mechanisms should be improved,and demand-side strategies need to be refined.关键词
