168例早产儿视网膜病变进展的影响因素研究
Analysis of factors affecting the progression of retinopathy of prematurity in 168 infants

Abstract
Objective To analyze the factors related to the natural regression and progression of Retinopathy of Pre-maturity(ROP),and to identify the elements influencing the progression of ROP lesions.Methods A retrospective analy-sis was conducted on 168 infants with ROP.The infants were divided into a progression group and a regression group based on their prognosis.A statistical analysis of relevant factors from both groups was performed,including:(1)General neo-natal characteristics:gestational age,birth weight,duration of oxygen therapy,and mechanical ventilation;(2)Neo-natal laboratory indicators:platelet count and hemoglobin level;(3)Maternal laboratory indicators and pregnancy com-plications:hemoglobin level,gestational diabetes,and hypertensive disorders of pregnancy.Results Among the 168 infants with ROP,43 cases(25.60％)progressed to a severe stage requiring surgical intervention,while 125 cases(74.40％)experienced spontaneous regression of lesions.Univariate analysis of the 168 ROP cases revealed statistically significant associations between ROP progression and the following factors:gestational age,birth weight,days of oxygen supplementation,days of mechanical ventilation,occurrence of infection,presence of sepsis,blood transfusion,and the mean platelet concentration within the first 4 weeks after birth(P＜0.05).Multivariate logistic regression analysis identi-fied the duration of oxygen therapy as an independent risk factor for ROP progression(OR＞1,P＜0.001).Conclusions The duration of oxygen exposure is an independent risk factor for the progression of ROP.A longer duration of oxygen thera-py is associated with a higher risk of ROP progression.Strict management of oxygen therapy duration in premature infants and those with ROP is essential to effectively prevent lesion progression and reduce the risk of severe ROP.关键词
