Abstract
[Objective]To explore the diagnostic value of mammography combined with serum carbohydrate antigen 153(CA153)and cytokeratin 19 fragment(CYFRA21-1)levels for early breast cancer.[Methods]A retrospective analysis method was used,147 suspected cases of breast cancer were collected from Sanmenxia Central Hospital between January 2024 and January 2025 as research samples.Based on the pathological diagnosis results,the subjects were divided into a malignant group(94 cases)and a benign group(53 cases).The imaging characteristics of mammography in breast cancer patients were analyzed,and the levels of serum CA153 and CYFRA21-1 were compared between the two groups.The diagnostic efficacy of mammography,serum CA153,and CYFRA21-1 levels,both individually and in combination,for early breast cancer was analyzed.[Results]Among the direct signs of breast cancer,there were 61 cases of calcification,8 cases of structural disorder,77 cases of mass.Among the indirect signs,there were 59 cases of star sign,13 cases of breast duct abnormality,18 cases of malignant halo,17 cases of breast vascular abnormality,1 case of local skin thickening or contraction,and 3 cases of nipple retraction.Compared with the benign group,the malignant group had higher levels of serum CA153 and CYFRA21-1,and the differences were statistically significant(P<0.05).The incidence of calcified masses,stellate sign,malignant halo,and vascular thickening in the malignant group was significantly higher than that in the benign group,with the difference being statistically significant(P<0.05).The individual diagnostic results of mammography,serum CA153,and CYFRA21-1 levels showed moderate consistency with pathological examination results(Kappa values=0.607,0.603,0.597),while the combined diagnostic results of various indicators showed high consistency with pathological examination results(Kappa value=0.783).The area under the reciever operating characteristic curve(AUC)value of the combination of mammography and serum CA153,CYFRA21-1 level detection for breast cancer diagnosis was higher than that of single detection,and the difference was statistically significant(P<0.05).[Conclusion]Mammography,serum CA153,and CYFRA21-1 levels all have certain diagnostic value for early breast cancer.The combination of these indicators can provide a higher value reference for clinical diagnosis of breast cancer and guide the formulation of subsequent treatment plans.关键词
