Abstract
[Objective]To explore the predictive value of peripheral blood interleukin-6(IL-6)combined with CD4+T lymphocyte count and CD8+programmed cell death protein 1-positive(PD-1+)T cell ratio for the disease outcome of pneumonia patients.[Methods]A retrospective collection of clinical data was conducted on 106 pneumonia patients admitted to Nanyang First People's Hospital from February 2022 to February 2024.Based on the patients'7-day outcomes(clinical efficacy),they were divided into a good outcome group(73 cases)and a poor outcome group(33 cases).The peripheral blood IL-6 combined with CD4+T lymphocyte count and PD-1+T cell ratio between two groups before treatment,3 days after treatment,and 7 days after treatment were compared.The predictive value of single and combined detection of the above indicators for the disease outcome of pneumonia patients was further analyzed.[Results]Intragroup comparison showed that in both groups,the peripheral blood IL-6 levels and the proportion of CD8⁺PD-1⁺ T cells gradually decreased over the treatment period,while the CD4⁺ T lymphocyte count gradually increased(all P<0.05).Intergroup comparison showed that at all time points,the peripheral blood IL-6 levels and the proportion of CD8 ⁺ PD-1 ⁺ T cells in the good outcome group were lower than those in the poor outcome group,while the CD4 ⁺ T lymphocyte count was higher than that in the poor outcome group(all P<0.05).Pearson correlation analysis showed that after 3 and 7 days of treatment,the peripheral blood IL-6 level in pneumonia patients was positively correlated with the proportion of CD8⁺PD-1⁺ T cells(r>0,P<0.05),and negatively correlated with the CD4⁺ T lymphocyte count(r<0,P<0.05).Receiver operating characteristic(ROC)curve analysis showed that the predictive value of combined detection of peripheral blood IL-6,CD4⁺ T lymphocyte count,and CD8⁺PD-1⁺ T cell proportion for disease outcome in pneumonia patients dynamically increased over the course of treatment,with the combined detection at 7 days after treatment having the optimal predictive performance.[Conclusion]The peripheral blood IL-6,CD4+T lymphocyte count,and CD8+PD-1+T cell ratio expression of pneumonia patients are closely related to disease progression,and combined detection has certain predictive value for patient disease progression.关键词
