乳腺DISCO-VIBRANT与常规DCE-VIBRANT动态增强MRI的图像质量对比研究
Comparative study of image quality of dynamic enhancement MRI between DISCO-VIBRANT and conventional DCE-VIBRANT for breast

Abstract
Objective:To compare and study the advantages and disadvantages of image quality of dynamic enhancement of two kinds of magnetic resonance imaging(MRI)for breast,which include Cartesian sampling-based k-space shared three-dimensional volumetric fast dynamic imaging(DISCO)that combined with volumetric imaging for breast assessment(VIBRANT)sequence(DISCO-VIBRANT)for breast,and conventional dynamic contrast-enhanced volumetric imaging(DCE-VIBRANT)for the breast,so as to provide basis for clinically rational application of DCE technique.Methods:A total of fifty-four patients with breast lesions who were clinically preliminarily diagnosed by People's Hospital of Lixin County from January 2024 to June 2025 were enrolled.According to preliminary findings of breast ultrasound and digital X-ray for lesions,the patients were divided into DISCO-VIBRANT group(n=26)and conventional DCE-VIBRANT group(n=28).The Pioneer 3.0T MRI scanner was adopted,and the images of the 10th phase of DISCO-VIBRANT,and the images of the 3rd phase of conventional DCE-VIBRANT were respectively selected.Regions of interest(ROIs)were placed at glandular tissue of the mammary gland,lesions,and fat areas to measure signal intensity and signal intensity of background.The signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)were calculated.Subjective evaluation of image quality was performed by two radiologists.Results:In the DISCO-VIBRANT group,the mean of signal intensity(SI)of lesions,glands,fats,background signals,SNR and CNR on images were respectively 975.74±453.13,413.56±118.12,76.85±25.65,2.77±1.16,167.30±74.09 and 217.18±187.53.The above indicators of the conventional DCE-VIBRANT group were respectively 1 441.17±336.80,508.16±143.57,190.48±49.47,10.90±3.47,50.11±17.79 and 93.14±40.30.The total sum of the four indicators(lesions of breast,glands,fat,and background signal)was 1468.91±493.03 in the DISCO-VIBRANT group,and that was 2 150.71±448.93 in the conventional DCE-VIBRANT group,and the difference was statistically significant(t=-5.319,P<0.05).The subjective scores[M(Q1,Q3)]of overall image quality,lesion resolution,differentiation between fat and glands,and the degree of helping diagnosis of the DISCO-VIBRANT group were respectively 4(4,5),4(4,4),4(4,5)and 4(4,5).The Kappa values of consistent assessment between two radiologists were respectively 0.743,0.612,0.772,and 0.625.The subjective scores of the above indicators were respectively 5(5,5),5(5,5),5(5,5),and 5(5,5)in the conventional DCE-VIBRANT group,and the Kappa values of them were respectively 0.781,0.711,0.781,and 0.650.All of subjective scores in the conventional DCE-VIBRANT group were higher than those in the DISCO-VIBRANT group,and there were higher consistency between two radiologists,and the range of Kappa values was from 0.612 to 0.781.The scanning times of the DISCO-VIBRANT group and the conventional DCE-VIBRANT group were respectively 3 min 36 s and 8 min 43 s.Conclusion:The DISCO-VIBRANT technique is superior to conventional DCE-VIBRANT in terms of scanning speed,noise control,and provision of quantitative parameters.However,conventional DCE-VIBRANT has higher spatial resolution,and has advantages in displaying breast ducts and small blood vessels of tumor.Clinical operation should select appropriate MRI dynamic contrast-enhanced technique according to diagnostic needs.关键词
