Serum Levels of Interleukins in Children with Mycoplasma Pneumoniae Pneumonia and their Relationship with Disease Severity

Abstract
Objective To investigate the serum interleukin(IL)levels in children with Mycoplasma pneumoniae pneumonia(MPP)and their relationship with the severity of the disease.Methods A retrospective analysis was conducted on 393 children with MPP who were treated at the Department of Pediatrics,the First Affiliated Hospital of Naval Medical University,from July 2023 to December 2024.According to the severity of the disease,they were divided into a severe MPP(SMPP)group(174 cases)and a non-severe MPP(NSMPP)group(219 cases).The general clinical data between the two groups were compared.Univariate and multivariate Logistic regression analyses were used to analyze the influencing factors of SMPP,and ROC curve analysis was performed to evaluate the diagnostic value of IL for SMPP.Results Compared with the NSMPP group,the SMPP group had a longer disease duration and higher levels of platelet count,IL-1β,IL-5,IL-6,IL-12,and IL-17A,while IL-2 levels were lower(P<0.05).Univariate Logistic regression analysis showed that disease duration,IL-12,and IL-17A were risk factors for SMPP(P<0.05);multivariate logistic regression analysis showed that disease duration,IL-12,and IL-17A were independent risk factors for SMPP(OR=1.358,95%CI:1.166-1.583,P<0.001;OR=1.428,95%CI:1.177-1.732,P<0.001;OR=1.221,95%CI:1.045-1.427,P=0.012).The prevalence of SMPP and the proportion of patients with pleural effusion and large pulmonary consolidation were higher in the high IL-12 and high IL-17A groups than in the low groups(P<0.001).ROC curve analysis showed that the areas under the curve for serum IL-1β,IL-2,IL-5,IL-6,IL-12,and IL-17A in diagnosing SMPP were 0.580,0.566,0.595,0.637,0.639,and 0.608,respectively,with serum IL-12 having the largest area under the curve.Conclusion Serum IL-12 and IL-17A are significantly elevated in children with SMPP and are independent risk factors.Early detection of IL-12 and IL-17A has certain auxiliary predictive value for evaluating the severity of MPP in children.关键词
