Abstract
Objective To evaluate the diagnostic value of the American College of Radiology contrast-enhanced ultrasound(CEUS)liver imaging reporting and data system(LI-RADS)for hepatocellular carcinoma(HCC)in different risk populations.Methods The clinical data of 538 patients with focal lesions in liver admitted to Jinhua People's Hospital from October 2018 to October 2024 were retrospectively collected.With reference to CEUS LI-RADS criteria,the patients were categorized into high-risk group and non-high-risk group based on HCC risk factors,and the lesions were graded.The surgical or biopsy pathology was used as the gold standard to evaluate the diagnostic value of CEUS LI-RADS in high-risk and non-high-risk populations.Results There were 309 cases in high-rish group and 229 cases in non-high-rish group.Using LR-5/M for diagnosing malignant tumors,the sensitivities were 90.2%and 94.7%,area under the curve(AUC)were 0.824(95%CI:0.752-0.895)and 0.807(95%CI:0.739-0.875),respectively.Using LR-5 for diagnosing HCC,sensitivities were 60.4%and 64.7%,specificities were 96.1%and 98.6%,positive predictive values were 93.9%and 78.6%,AUC were 0.783(95%CI:0.729-0.836)and 0.816(95%CI:0.676-0.957),respectively.Using LR-M for diagnosing other hepatic malignancies(OM),the sensitivities were 94.0%and 93.3%,AUC were 0.850(95%CI:0.806-0.895)and 0.809(95%CI:0.746-0.871),respectively.Conclusion CEUS LI-RADS LR-5 is highly valuable for HCC diagnosis in high-risk group,while LR-M shows high sensitivity in differentiating HCC from OM across both groups.关键词
