食管静脉曲张套扎联合硬化治疗晚期血吸虫病的临床研究
Clinical Study of Esophageal Variceal Ligation Combined with Sclerotherapy in the Treatment of Advanced Schistosomiasis

Abstract
Objective:To explore the effect of esophageal varices ligation combined with sclerotherapy in the treatment of advanced schistosomiasis complicated with esophageal varices.Method:100 patients with advanced schistosomiasis complicated with esophageal varices were randomly divided into ligation group(33 cases,esophageal varices ligation),sclerotherapy group(33 cases,esophageal varices sclerotherapy)and combination group(34 cases,upper esophageal ligation combined with lower esophageal sclerotherapy).The clinical efficacy,varices diameter,portal vein hemodynamics,adverse reactions and complications were compared.Result:The total effective rate of the combined group was higher than the ligation group and the sclerotherapy group,and the number of times of treatment required in the combined group was less than the ligation group and the sclerotherapy group.1 month,3 months,6 months and 12 months after treatment,the varicose vein diameter in the combined group was smaller than the ligation group and the sclerotherapy group,the portal vein pressure(PVP)in the combined group was lower than the ligation group and the sclerotherapy group,and the portal vein blood flow velocity(PVV)in the combined group was higher than the ligation group and the sclerotherapy group.The incidence of adverse reactions and complications in the combination group was lower than that in the sclerotherapy group,and the difference was statistically significant(P<0.05).Conclusion:Upper esophageal ligation combined with lower esophageal sclerosing agent injection in the treatment of advanced schistosomiasis complicated with esophageal varices is effective,which can reduce the diameter of varices,improve portal vein hemodynamics,and reduce the risk of adverse reactions and complications.关键词
