应用风药治疗糖尿病性黄斑水肿的理论探讨
Theoretical discussion on wind-dispelling medicine in the treatment of diabetic macular edema

Abstract
Wind-dispelling medicines refer to a class of drugs that possess characteristics such as uplifting,dispersing,regulating,and soothing.They often have a light and pungent nature,with a predominant spicy flavor,light texture,and ease of dispersion,capable of guiding the medicine upward,penetrating to dispell pathogens,and promoting blood circulation.From a modern pharmacological perspective,wind-dispelling medicines often contain volatile components and bioactive substances,demonstrating a clear upward and outward trend in the human body,which gives them a significant advantage in treating diseases of the head and face.Diabetic macular edema(DME),as one of the most threatening complications of diabetic retinopathy,is primarily treated in Western medicine with anti-vascular endothelial growth factor(VEGF)drugs injections and laser photocoagulation.Although these methods can temporarily alleviate symptoms,they have limitations such as high treatment costs,the need for repeated injections,and inability to completely cure the condition.In this context,the integrative medicine treatment model shows unique advantages,particularly with the application of wind-dispelling medicines in traditional Chinese medicine(TCM).From the perspective of TCM theory,the core pathogenesis of DME lies in the dysfunction of the liver,spleen,and kidney,leading to disturbances in the metabolism of Qi(气),blood,and body fluids.Dysfunction in the liver's ability to regulate leads to Qi stagnation,while the spleen's inability to transport results in the retention of dampness,and insufficient kidney Qi leads to impaired transformation,causing damage to the eye vessels,retention of dampness,and ultimately resulting in edema in the macula.In response to this complex pathogenesis,wind-dispelling medicines have functions such as uplifting and dispersing Yang(阳),soothing the liver and relieving depression,warming Yang and invigorating the spleen,clearing channels and dispelling wind,and guiding drugs to meridians.These comprehensive effects enablee wind-dispelling medicines show broad application prospects in the treatment of DME.This article is based on the fundamental theoretical system of TCM,by systematically reviewing ancient and modern literature and clinical experiences,exploring the theoretical basis and mechanisms of wind-dispelling medicines in treating DME from multiple dimensions,including related organs,Qi,blood,body fluids,and meridian circulation,aims to offer novel insights for the treatment of DME.关键词
