Discussion on the diagnosis and treatment of pediatric enuresis based on the"three roots and two unifications"theory

Abstract
Based on the"three roots and two unifications"(三本二统)theory for treating deficiency patterns,this paper systematically elaborates on the traditional Chinese medicine(TCM)diagnostic and treatment system for pediatric enuresis,focusing on the lungs,spleen,and kidneys.It explicitly proposes the core pathogenesis:"When metal fails to govern water,bladder restraint is impaired;when the central earth fails to transport,the regulation and movement of dampness become abnormal,when fire fails to warm the lower body,bladder Qi(气)transformation loses its authority."In response to the current clinical tendency of"emphasizing warming and consolidation of the lower Jiao(焦)while neglecting the regulation of the upper and middle Jiao",and grounded in the holistic concept of TCM,this paper emphasizes the unity of the triple Jiao and the synergy among the organs.The coordinated functioning of the lungs,spleen,and kidneys constitutes the core mechanism for maintaining the orderly metabolism of body fluids.The pathological root of pediatric enuresis is closely related to the impaired diffusion and descent of lung Qi in the upper Jiao and the dysfunctional transportation and transformation of spleen earth in the middle Jiao.Solely focusing on tonifying the kidneys and securing containment while neglecting the regulation of the lungs and spleen can easily lead to limited therapeutic efficacy or prolonged and recurrent conditions.Therefore,from the perspective of"synergizing the Three roots and regulating the two unifications concurrently",this paper systematically details the pattern-differentiated treatment strategies for pediatric enuresis,employing the method of"warming,tonifying,consolidating,containing,transforming Qi,moving water,and arresting enuresis".For cases characterized by metal depression and water coldness with impaired Qi transformation,the approach involves opening blockages and promoting the source,allowing metal and water to mutually nourish in order to facilitate Qi and secure the spring.For patterns of earth decline and water overflow with failure of the restraining"gate",the method is to fortify the earth and warm the water,utilizing the mutual generation of fire and earth to consolidate the bladder and arrest enuresis.When two unifications are impaired and Qi transformation is abnormal,the treatment coordinates the triple Jiao and balances Yin(阴)and Yang(阳)to strengthen the root and reduce excessive urination.The modification of formulas in this paper reflects the compatibility principle of"treating the upper without forgetting the middle and lower,and incorporating diffusion and movement when warming the lower".For instance,lung-diffusing formulas are supplemented with kidney-consolidating ingredients,and warm-tonifying prescriptions incorporate spleen-regulating medicinals.The aim is to provide a clinical approach that not only emphasizes warming and tonifying the lower source to address the manifestations but also prioritizes regulating and dredging the upper and middle Jiao to resolve the root cause.This method achieves the dual objectives of addressing both the source and the flow,as well as treating both the root and the branch,thereby offering systematic insights and methodological references for the pattern-differentiated treatment of pediatric enuresis.关键词
