基于虚、滞、瘀探讨脑小血管病相关性头晕的病机及治疗
Exploring the pathogenesis and treatment of dizziness associated with cerebral small vessel disease from the concepts of deficiency,stagnation,and blood stasis

Abstract
Cerebral small vessel disease(CSVD)-related dizziness falls within the scope of"vertigo"in traditional Chinese medicine(TCM),which significantly impacts patients'quality of life.Western medicine primarily focuses on symptomatic treatment,with limited long-term efficacy.Professor WU Dahua posits that the core pathogenesis of CSVD-related dizziness lies in"deficiency,stagnation,and stasis",with kidney essence deficiency and marrow sea malnourishment as the fundamental cause.Qi(气)stagnation,phlegm stasis,and obstruction of the brain collaterals constitute the core pathological processes,with these three factors interacting and interconverting throughout the disease course.In clinical practice,the treatment emphasizes"tonifying the kidney and replenishing essence to consolidate the root",supplemented by"soothing the liver and regulating Qi,resolving phlegm,and activating blood circulation to dredge collaterals".By integrating typical cases and cross-referencing mechanisms from both TCM and Western medicine,this study systematically summarizes Professor WU's diagnostic and therapeutic experience,providing theoretical foundations and practical references for TCM prevention and treatment of this condition.关键词
