基于抗凝管理平台的延续性抗凝管理在心脏瓣膜置换术后患者中的应用
Application of continuous anticoagulation management based on anticoagulation management platform in patients after heart valve replacement

Abstract
Objective To investigate the application effect of continuous anticoagulation management based on the anticoagulation management platform in patients after cardiac valve replacement surgery.Methods Sixty patients who underwent heart valve replacement surgery and received Warfarin anticoagulation therapy from May 2021 to May 2023 were selected and divided into a control group(n=30)and a study group(n=30)according to the random number table method.The control group received conventional anticoagulation management,while the study group received continuous anticoagulation management based on the anticoagulation management platform on the basis of the control group.The mastery of anticoagulation knowledge,the rate of achieving therapeutic international normalized ratio(INR)range(INR achievement rate),treatment compliance,quality of life[European Five-Dimensional Quality of Life Scale(EQ-5D)score],and the incidence of complications were compared between the two groups.Results At 3 months after the surgery,the INR achievement rate in the study group was 96.67％(29/30),which showed no significant difference compared with the 83.33％(25/30)in the control group(P>0.05).At 1 year after the intervention,the study group scored higher than the control group in terms of anticoagulation knowledge mastery,treatment compliance,and overall score(P<0.05).At 1 year after the intervention,the EQ-5D scores of each dimension and the total score of the study group were lower than those of the control group(P<0.05).There was no significant difference in the total incidence of complications between the two groups during the intervention period(P>0.05).Conclusion Continuous anticoagulation management based on the anticoagulation management platform can enhance the compliance of patients after cardiac valve replacement with Warfarin anticoagulation therapy,increase the INR achievement rate,improve their mastery of anticoagulation knowledge,and enhance their quality of life.关键词
