糖尿病视网膜病变所致玻璃体积血患者术后视力改善的相关因素分析
Analysis of factors associated with postoperative visual improvement in patients with diabetic retinopathy-related vitreous hemorrhage

Abstract
Objective To identify factors associated with postoperative visual improvement in patients with dia-betic retinopathy complicated by vitreous hemorrhage.Methods A retrospective review of the medical records from pa-tients with proliferative diabetic retinopathy(PDR)and fundus-obscuring vitreous hemorrhage who received vitrectomy be-tween January 2019 and April 2022 was conducted.Significant postoperative visual improvement was defined as the best-corrected visual acuity(BCVA)of 0.3 logMAR or better at 3-month postoperatively.Ocular and systemic data were col-lected and investigated using logistic regression analysis.Results A total of 162 eyes from 162 patients were enrolled.The BCVA significantly improved from(1.37±0.65)logMAR preoperatively to(0.61±0.46)logMAR at the 3-month follow-up.Significant postoperative visual improvement was associated with intact ellipsoid zone(aor,2.384;95%CI:1.120～5.076;P=0.024),absence of vitreous fibrovascular proliferation(aor,2.957;95%CI:1.325～6.600;P=0.008),good baseline BCVA(aor,0.433;95%CI:0.247～0.761;P=0.004),and less central foveal thickness(aor,0.997;95%CI:0.994～0.999;P=0.024).Conclusions Vitrectomy for PDR with dense vitreous hemorrhage leads to overall improved vision.How-ever,pre-existing macular structural abnormalities and risk factors may limit the extent of visual improvement.关键词
