胃部超声在结核病手术患者围术期的应用探索
Exploration of the Application of Gastric Ultrasound in the Perioperative Management of Patients Undergoing Tu-berculosis Surgery

Abstract
Objective To explore the preoperative gastric emptying function of tuberculosis patients undergoing elective surgery by gastric ultrasound,analyze its influencing factors,and provide a reference for the establishment of their preoperative as-sessment system.Methods A total of 250 eligible tuberculosis patients scheduled for elective general anesthesia were enrolled in the study.Preoperative data were collected,and gastric ultrasound was performed before anesthesia to qualitatively and quantitative-ly assess gastric contents.The rates of gastric fullness,incidence of reflux aspiration,and anesthesia strategies were documented.Results Of the 250 enrolled patients,three did not complete the gastric ultrasound examination.Thirty patients(12.1%)exhibi-ted gastric fullness,with all gastric contents being liquid.217 patients had empty stomachs.Univariate Logistic regression analysis indicated that age,female gender,duration of fasting and fluid restriction,7-point global overall symptom scale(GOSS)≥10 points,and anti-tuberculosis drug regimens for drug-resistant tuberculosis(DR-TB)might be associated with delayed gastric emptying.Multivariate Logistic regression analysis revealed that fasting duration,GOSS score≥10 points,and DR-TB medication regimens were independent risk factors for delayed gastric emptying in tuberculosis patients.Conclusion Gastric ultrasound examination is feasible and necessary for observing and measuring gastric contents in tuberculosis patients.GOSS score≥10 points,and DR-TB treatment regimens represent significant risk factors for delayed gastric emptying in TB patients.Individualized fasting time and rou-tine gastric ultrasound examination for patients with high-risk factors before surgery can provide anesthesiologists with individual-ized anesthesia management decision-making basis,thereby reducing the risk of reflux aspiration and improving perioperative safety.关键词
