老年食管癌根治术后患者症状群与生活质量相关性的纵向研究
A Longitudinal Study on the Correlation Between Symptom Clusters and Quality of Life in Elderly Patients Follow-ing Radical Esophagectomy for Esophageal Cancer

Abstract
Objective To investigate the symptom clusters and quality of life in elderly patients with esophageal cancer at different time points after radical resection,and to analyze the correlation between them,so as to provide a reference for dynamic management of symptom clusters and improvement of quality of life in patients.Methods A total of 201 elderly patients who un-derwent radical resection of esophageal cancer in the thoracic surgery department of our hospital Province from February to June 2024 were selected as the research objects.The general information questionnaire,the esophageal cancer symptom assessment scale and the Functional Assessment of Cancer Therapy(FACT-G)were used to investigate the patients at 7 days(T1),1 month(T2),3 months(T3)and 6 months(T4)after surgery.Results At T1 and T2,patients had psychological,fatigue-related,eating-related,energy deficiency and somatic symptom clusters,and at T3 and T4,the somatic symptom clusters were reduced.While the composi-tion of the psychological symptom cluster remained stable across time points,the composition of the other clusters varied.The sever-ity of fatigue-related symptom clusters and energy deficiency symptom clusters were negatively correlated with the total score and the scores of each dimension of quality of life at four time points(P＜0.05).The severity of psychological symptom cluster was negatively correlated with the total score and dimension scores of quality of life at T1 to T3(P＜0.05).The severity of eating-relat-ed symptom clusters was negatively correlated with the total score and each dimension score of quality of life at T2 to T4(P＜0.05).Conclusion Elderly patients experience multiple symptom clusters from 7 days to 6 months after radical esophagectomy,with both the internal composition and severity of these clusters showing dynamic changes.The severity of symptom clusters is nega-tively correlated with patients'quality of life.Medical staff should implement dynamic management of symptom clusters in elderly patients with esophageal cancer after radical resection.关键词
